Patient Centered Medical Home



As mentioned in A History of the HCHC, in 2011 the HCHC began the process of becoming a Patient Centered Medical Home recognized practice.
Patient Centered Medical Home is a program for improving primary care by centering care around patients, team work, and tracking care over time across various settings.  The program focuses on creating relationships between patients, their physicians, and if necessary the patient’s family.
In order to become a recognized practice of this program, the HCHC must score at least 50% in the following categories:
Access during office hours
Use data for population management
Care management
Support self-care process
Track referrals & follow-up
Implement continuous quality improvement
